  • Choose one way to fill out forms:
    • Sign up for the MyChart patient portal and fill out forms online. To sign up for MyChart, visit orthovirginia.com/mychart. OR
    • Arrive 20 minutes early for your appointment.
  • Bring your insurance card, ID, and a list of any medications you are currently taking.
  • Masks are optional. You are welcome to wear a mask, and if you ask the staff and providers who work with you will wear a mask as well.
  • Patients under age 18 must have a parent or guardian with them.
  • A late fee will be charged for appointments cancelled less than 24 hours before the scheduled time unless you are canceling due to the flu or flu-like symptoms.
  • Call to reschedule your appointment if you have the flu or flu-like symptoms. You will not be charged for the missed visit.

The provider you are having the appointment with must be licensed in the state you are physically located at the time of a telemedicine visit. State licensure and provider participation varies. A location verification will be required to begin a telemedicine appointment.
